Please bring your insurance card, copy of discharge instructions, eye ware, hearing aid, personal hygiene toiletries, loose clothing supportive shoes for rehabilitation. Please DO NOT bring valuable items that may be damaged or lost.

Patients may need a copy of their medical records. To obtain a copy please submit the form below to our Health Information Staff via mail or fax at 248-338-5429. You will be notified of any fees, if applicable before releasing of record. For any questions regarding the status of your request, contact us at 248-338-5430
